McNEIL HIGH SCHOOL
PROJECT GRADUATION 2010
Participation Agreement
FORM DUE MAY 14th
Project Graduation is a drug-free, alcohol-free celebration for a senior that takes place the night of graduation. It is a lock-in at the school to ensure that seniors have a safe graduation night filled with great memories. It is funded by parents and community members.
I understand that I will arrive at the party between 10:00 pm and 11:00 pm on Saturday, May 29, 2010 at the McNeil High School Cafeteria. I further understand that the doors will be locked promptly at 11:00 pm and that no one will be allowed to enter the party after that time. The party will end at 5:00 am on Sunday morning. Any emergencies during the evening will be handled by Project Graduation security.
I understand that I will NOT be allowed to leave and will be required to stay the entire length of the party until 5:00 am. The only way I can leave before that time is if the parent/guardian who signs this form comes to the school and signs me out. If circumstances necessitate that I must leave based on an emergency, a phone call will be made to the parent/guardian listed on this form and he/she will come to the school and sign me out. If I leave before the party is over, I will not be eligible for any prize drawings that occur.
I understand that I must be substance-free. This is a zero-tolerance party. NO ALCOHOL, NO DRUGS AND NO TOBACCO PRODUCTS. All school rules (including dress code) are applicable. I am aware than an officer will be in attendance and legal action will be taken if applicable.
I understand that I must wear my Project Graduation T-shirt and bring my McNeil ID (or other photo ID) without exception. The T-shirt and my ID are my ticket in. The only other items I may bring with me are a disposable camera (with my name on it) and a cell phone. I will not bring expensive cameras into the party. If I bring a cell phone, Project Graduation will not be liable if it is broken or lost. I am not allowed to bring backpacks, purses, food, drinks or anything else in with me. Everything will be provided for me. If I bring any of these items, they will be collected and returned to me at 5:00 am.
I understand that this party is free. It is a gift given to me by my parents, teachers and community. It is in recognition of my accomplishments and achievements.
By signing below, I confirm that I have read this agreement and understand the rules as stated.
